The LPN for the Residential Rehabilitation Treatment Program (RRTP) functions as a team member under the guidance of the Registered Nurse (RN) and RRTP Nurse Manager. Incumbent must be self-directed and able to function with minimal direction, communicating proactively and effectively with other members of the multidisciplinary team. **If you are a current permanent VA Employee, please apply to announcement CBTF-11077994-21-BNS-GroupF.** Duties include, but not limited to: The Licensed Practical Nurse is responsible for completing clinical reminders and medication reconciliation, providing medical assistive care, and providing administrative support when needed. The incumbent demonstrates flexibility in taking assignments and completes a comprehensive system of tasks that are specific to RRTP. The incumbent must demonstrate the ability to observe, identify, and respond to the patient's needs for nursing care, including, but not limited to, medication administration, patient education, specimen collection including various urologic and breathalyzer procedures, capillary blood glucose testing, equipment assisted care, evaluating for suicide risk, and recognizing deviations from normal including monitoring for signs of substance use and withdrawal. The incumbent is also responsible for patient education, safety protocols including room checks and contraband searches, milieu management activities, and documentation. She/he performs age and culturally appropriate care, uses techniques to appropriately de-escalate disruptive patients, and appropriately participates in emergency response activities. The RRTP LPN actively contributes to process improvement activities on the unit. General responsibilities include: Assist the RN to provide a therapeutic environment to Veterans with substance use disorders and other mental health conditions Assist RN to establish interventions to promote health and evidence-based psychological programming Establishes constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills. Conduct effective communication in the provision of safe clinical care Assists Veterans with their care needs, completes medical history and documents all clinical care information in CPRS Performs a range of direct care duties that include vital signs, oxygen saturation, height, weight and POC blood glucose checks Practice according to policy and procedure incompliance with The Joint Commission and Commission on Accreditation of Rehabilitation Facilities (CARF) Medication administration Completes safety rounds Assist with transporting Veterans to community activities The LPN assists the RN with admissions, discharges, leading groups and various activities as needed Work Schedule: Full-time to include days, evenings, nights, weekends and holidays Financial Disclosure Report: Not required
