The Case Management Education Team Lead handles and overseas the onboarding and education of the DUHS Case Management and Clinical Social Work Staff.
Functions as a Case Management subject matter expert.
Develops a preceptor-training program to facilitate day-to day orientation needs.
Oversees the orientation and onboarding of new employees and, when necessary re-education of existing employees.
Develop and maintains orientation resource documents for the CM and CSW roles.
Conducts routine audits to identify what is going well and opportunities to improve performance or processes.
Keeps CM Leadership informed of orientee's progress and gaps in knowledge and process.
In conjunction with appropriate CM Leadership, meets routinely with orientees to provide feedback identified from routine audits and preceptor feedback.
Maintains documentation of at least weekly communication with preceptor s and team-leads to stay abreast of orientees daily progress.
Models both intra and interdepartmental communication and collaboration.
Manage and facilitate EPIC training for the CM and CSW workflows, including ensuring scheduling of CEPD classes.
Works closely with CM EPIC super users to identify gaps in training and opportunities to re-educate.
In conjunction with the CM leadership Team, develops, presents or secures a presenter for Case Management/Clinical Social Work education topics both internal to the department as well as to external audiences (e.g. Providers).
Participates in staff meeting at all three hospitals as often a schedule permits.
Promotes an environment of interdepartmental collaboration across the DUHS CM departments.
Provides routine feedback to CM leadership to assist in the development of the 30-60-90 reviews
In conjunction with the CM Directors, develops workflow documents in response to identified needs.

BSN or MSW required
3 years of relevant experience
Degrees, Licensures, Certifications
Requires Case Management Certification (ACM, CCM or ANCC) within 2 yearsor of hire. BSN required and must have current or compact RN licensure in state of NC. MSW
